TAKE WITH YOU TO YOUR STUDENT’S SCHOOL:
- Original birth certificate (Kindergarten, first graders, and students new to the district).
- Immunization Record Form FL680.
- Physical Exam Form DH 3040 (completed within the past year).
- Any record or report cards from the previous school, including IEP.
- Take your 2 proofs of address that you provided in the application.
- If your child needs a bus, please go to our website and click on the Transportation button to register for a bus.
